Who pays more
- Higher income: IRMAA adds a surcharge if your income from two years before is above the first line ($109,000 single, $218,000 joint, in 2026). IRMAA calculator.
- Late sign-up: 10% more for each full 12 months you could have had Part B but didn't, usually for life. Enrollment periods.
Hold harmless: your check can't shrink
If Part B is taken out of your Social Security check and you don't pay IRMAA, the hold-harmless rule keeps the Part B increase from being larger than your COLA increase. In 2027 the COLA is 3.6%. See your 2027 benefit.
Questions
What is the Medicare Part B premium for 2027?
It hasn't been announced yet (as of November 6, 2026). CMS usually announces it in November. The 2026 standard premium is $202.90 a month and the 2026 deductible is $283.
Will the Part B increase eat my COLA?
Part of it, for most people. But if Part B is taken out of your Social Security check and you don't pay IRMAA, the hold-harmless rule stops your Part B premium from rising by more than your COLA, so your net check can't go down.
Who pays more than the standard Part B premium?
People with higher income pay IRMAA, an extra charge based on their tax return from two years before. People who signed up late can pay a late penalty. Some people not protected by hold harmless can also pay more.
